Revamp Home Care
Because we have you in mind, we have packaged a special product to facilitate patients who are unable to access inpatient care. We have done the planning for you. The package includes:
- Nursing Care: We provide trained Nursing Assessments for all your needs
- Physician Coverage: Our Physician is accessible by phone and virtual consultations. Emergency visits are arranged separately.
- Physical Therapy: We will provide the best person for your needs
- Speech Therapy: Available based on your needs.
- Swallow Therapy
How it works:
Upon hospital discharge, our Physician will perform a medical assessment and make recommendations for therapy. Our occupational therapist will provide a home assessment to make recommendations for a safe and functional home environment. Once everything is in place we will provide the nursing staff as well a therapy/physician schedule. Once the patient has achieved a certain level of recovery, outpatient care, including Occupational therapy is recommended at REVAMP outpatient services.
